Support Worker, Look Ahead Care Supportand Housing in Hammersmith & Fulham.

Job description:
Looking for a new role where you can make a difference every day? We’re looking for a kind, compassionate and resilient Support Worker to join our Mental Health service in Hammersmith & Fulham

Shift patterns:
Early 7.30am-3.30pm & Late 2PM-10pm

We work all days of the year including bank holidays and weekends

Any 5/7

£23,026.00 per annum, working 40 hours per week, plus 25 days Annual Leave, Pension schemes, staff discounts, healthcare and career progression.

We work across mental health, homelessness, young people and learning disabilities so there are plenty of opportunities to grow and progress your career with us.

We’re looking for Support Workers within Hammersmith & Fulham Mental Health Service support customers to help them to develop life skills and increase independence. A wide range of co-produced activities take place across the service, designed to empower, and improve the daily living skills of customers. Support Workers in the service work across all 4 projects; working with customers with varying levels of support needs. Many of the customers have complex needs including dual diagnosis and support workers are expected to liaise with other agencies within the borough to best support customers.

Skills, Knowledge and Experience:
Essential:

  • NVQ Level 2 or equivalent or experience within charity/social care sector
  • Challenging situations- Anti-social behaviour
  • Understanding and empathy with client group. Drug users and Alcoholics

Desirable:

  • Degree
  • Experience with complex needs 1 years- MH and Complex. Adults- would accept young people MH experience.

Key Responsibilities:

  • If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer
  • Support customers to maintain their tenancies
  • Support to budget and prevent rent arrears
  • Assess their practical and emotional needs to incorporate in their support plan
  • To develop support plans and risk management plans
  • Carry out weekly health and safety checks
  • Carry out security duties / welfare checks to ensure the safety of the customers and premises
  • Monitor the CCTV throughout the shift
  • Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
  • Report any observations relating to customers welfare
  • Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
  • Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times
